For Survey Participants
The Sinai Urban Health Institute, along with several community partners, is conducting the largest door-to-door health survey in Chicago and we need your assistance. Your household was randomly selected to be included because you live in one of the 9 communities in our survey.
Why should I participate?

Your opinions are valuable to us and will be used to improve the health and wellbeing of your community.

The questions on this survey were reviewed and revised by a committee of community residents.

We will work with community members throughout every step of this process to ensure that survey results are used to benefit your community.

Findings from our last survey resulted in a variety of community programs to address health issues such as asthma, diabetes, breast cancer, and smoking.
How do I sign up for the survey?

Interviewers will be coming to your community soon to select households. Homes will be chosen randomly by using a process similar to a lottery.

If your household is chosen, one or more household members will be randomly selected to participate in the interview and will receive $50 for completing it. If your household is eligible for a second part of the interview, you will receive an additional $25 for completing the second part.
